What Is Childhood Apraxia of Speech?

Childhood Apraxia of Speech is a speech disorder in which a child has difficulty planning and programming the precise movements needed to produce speech sounds. The muscles of the lips, tongue, and jaw may not be weak, but the brain has difficulty organizing their movements accurately and consistently.

A child with CAS may know exactly what they want to say but struggle to get the sounds in the correct sequence.

CAS is different from a simple articulation difficulty. While some children may have trouble producing particular sounds, children with CAS may experience broader difficulties with speech movement planning, sequencing, timing, and consistency.

Common Signs to Watch For

Speech is difficult to understand, especially for unfamiliar listeners
The same word may sound different each time
Difficulty imitating sounds, words, or phrases
Longer or more complex words are especially challenging
Errors may increase as words become longer
Pauses or hesitations occur between sounds or syllables
Difficulty sequencing sounds and syllables
Speech may sound slow, effortful, or unusual
Inconsistent vowel or consonant errors
Difficulty transitioning smoothly from one sound to another
Limited spoken vocabulary despite wanting to communicate

Some children with CAS may also have difficulty with other aspects of communication. However, not every child with unclear or delayed speech has CAS. A detailed speech-language assessment is important for accurate identification.

What Causes Childhood Apraxia of Speech?

CAS can have different causes. In some children, it may be associated with:

Neurological conditions or developmental differences
Genetic conditions
Brain injury or other neurological events
Certain syndromes or developmental disorders

In many children, however, the specific cause is not identified. CAS is not caused by laziness, lack of intelligence, or simply not practising enough.

How Is CAS Evaluated?

A speech-language professional may assess:

Speech sound accuracy
Consistency of speech errors
Ability to imitate sounds and words
Movement and sequencing of speech sounds
Production of vowels and consonants
Syllable and word patterns
Speech rate and rhythm
Stress and intonation patterns
Ability to produce increasingly complex words
Connected speech and overall intelligibility

The clinician may compare how the child performs when producing single sounds, syllables, words, phrases, and spontaneous speech.

A hearing evaluation may also be recommended because hearing difficulties can affect speech and language development.

Why Early Identification Matters

When a child is difficult to understand, communication can become frustrating at home, school, and during social interactions. Increasing consistency of speech production
Building from simple sounds and syllables to words and phrases
Improving speech intelligibility
Developing appropriate speech rhythm and stress
Supporting functional communication in everyday situations
Using alternative or augmentative communication when appropriate
