Measuring Attention to Sound: Understanding the P300 Test

Not all hearing tests measure what the ear picks up — some measure what the brain actively notices and attends to. The P300 Test, a cognitive auditory evoked potential, does exactly this, revealing how well the brain recognizes meaningful sound.


What Is the P300 Test?

P300 is a "cognitive" evoked potential, depending on active mental engagement rather than the sound itself. It's measured using an "oddball paradigm" — frequent, repetitive tones interspersed with occasional rare, different tones. The brain generates a distinctive positive response around 250–500 milliseconds after detecting the rare tone, reflecting attention and cognitive processing of sound.


Test Specifications

  • Duration: 45–60 min
  • Equipment: Evoked potential system with scalp electrodes, insert earphones
  • Preparation: Clean scalp; patient awake, alert, and actively attentive to the task
  • Setting: Quiet, electrically shielded room at the clinic

Procedure of the Test

  1. Scalp cleaned; electrodes attached with adhesive gel at standard positions.
  2. Patient seated, awake and alert, instructed to actively listen for the "rare" sound among frequent tones.
  3. Frequent standard tones played, interspersed with occasional rare target tones, via insert earphones.
  4. Patient counts or mentally notes each rare tone to maintain attention.
  5. Electrodes detect the response in the 250–500 millisecond window following each rare tone.
  6. Responses averaged and compared against expected latency and amplitude values.

  • Assesses cognitive processing and attention to auditory stimuli, beyond simple detection

What Will the Result Show?

A waveform showing presence, timing (latency), and size (amplitude) of the P300 response to rare tones, compared against expected normal values for age.


Interpretation

  • Normal: "P300 response present, latency and amplitude within normal limits — consistent with typical cognitive processing and attention."
  • Delayed Latency: "Prolonged P300 latency — may suggest slowed processing speed; correlate with history and consider further evaluation."
  • Reduced Amplitude: "Reduced P300 amplitude — may indicate reduced attention or engagement; further assessment recommended."
  • Absent Response: "No measurable P300 response — warrants evaluation to rule out attention or cognitive processing concerns."
