Listening Beyond the Brainstem: Understanding the Middle Latency Response (MLR) Test

Sound doesn't stop being processed once it reaches the brainstem — it travels onward to deeper brain structures and eventually the auditory cortex. The Middle Latency Response (MLR) Test captures this next stage, revealing how sound is processed beyond the level assessed by standard ABR testing.


What Is the MLR Test?

MLR is an auditory evoked potential test recording tiny signals from the thalamus and auditory cortex, occurring roughly 10 to 80 milliseconds after a sound — later than brainstem responses (ABR) but before longer-latency cortical responses. Scalp electrodes detect these signals in response to clicking or tone-burst sounds.


Test Specifications

  • Duration: 45–60 min
  • Equipment: Evoked potential system with scalp electrodes, insert earphones
  • Preparation: Clean scalp; patient relaxed and awake but still (less reliable during sleep, unlike ABR)
  • Setting: Quiet, electrically shielded room at the clinic
  • Age Group: Older children through adults (less reliable in infants)

Procedure of the Test

  1. Scalp cleaned; electrodes attached with adhesive gel at standard scalp positions.
  2. Patient seated comfortably, asked to remain relaxed, awake, and still.
  3. Clicking or tone-burst sounds presented through insert earphones, one ear at a time.
  4. Electrodes detect the brain's response in the 10–80 millisecond window following each sound.
  5. Responses averaged over many repetitions to produce a clear waveform.
  6. Wave latencies and amplitudes analyzed and compared to normal values.

  • Assesses auditory processing at the thalamus and cortex, beyond the brainstem
  • Supports threshold estimation, particularly at lower frequencies where ABR is less reliable
  • Evaluates suspected central auditory processing concerns
  • Assists cochlear implant candidacy evaluation by confirming higher pathway integrity

What Will the Result Show?

A waveform showing distinct peaks in the 10–80 millisecond range, with timing (latency) and size (amplitude) compared against expected values for age.


Interpretation

  • Normal: "MLR waveforms present with latencies and amplitudes within normal limits — consistent with intact processing at the thalamic and cortical level."
  • Absent/Reduced Response: "Reduced or absent MLR components — may indicate central auditory processing concerns; further evaluation recommended."
  • Delayed Latencies: "Prolonged wave latencies — suggestive of possible delay in central pathway conduction; correlate with clinical history and other findings."
  • Asymmetric Findings: "Significant difference between ears — warrants further evaluation to rule out one-sided pathway involvement."
