The Brain's Final Word on Sound: Understanding the Late Latency Response (LLR) Test

Hearing isn't complete until sound reaches the thinking, conscious part of the brain. The Late Latency Response (LLR) Test, also called the Cortical Auditory Evoked Potential (CAEP), captures this final stage — showing whether the cortex is genuinely detecting sound.


What Is the LLR Test?

LLR is an auditory evoked potential test recording brain activity from the auditory cortex, occurring roughly 50 to 300+ milliseconds after a sound — much later than ABR (brainstem) or MLR (thalamocortical) responses. Since it reflects true cortical detection, it's especially useful for confirming whether sound, including speech, is registered by the brain, aided or unaided.


Test Specifications

  • Duration: 45–60 min
  • Equipment: Evoked potential system with scalp electrodes, insert earphones or sound field speakers
  • Preparation: Clean scalp; patient awake, relaxed, and attentive (requires wakefulness, unlike ABR)
  • Setting: Quiet, electrically shielded room at the clinic
  • Age Group: Infants through adults, including hearing aid/cochlear implant users

Procedure of the Test

  1. Scalp cleaned; electrodes attached with adhesive gel at standard positions.
  2. Patient sits comfortably, awake and relaxed, sometimes engaged in a quiet visual task.
  3. Speech sounds or tone bursts presented through earphones or speakers, unaided and/or aided as needed.
  4. Electrodes detect the response in the 50–300+ millisecond window following each sound.
  5. Responses averaged into a clear waveform; latencies and amplitudes compared to normal values.

  • Assists threshold estimation when behavioral testing is unreliable

What Will the Result Show?

A waveform showing cortical response peaks in the 50–300+ millisecond range, with presence, timing (latency), and size (amplitude) compared to expected values, unaided and aided where relevant.


Interpretation

  • Normal/Present: "Cortical response present with latency and amplitude within normal limits — sound detected at cortical level."
  • Absent (Unaided): "No cortical response at presented levels — supports further evaluation and appropriate amplification."
  • Present (Aided): "Cortical response present with amplification — supports that current settings provide audible access to sound."
  • Delayed/Reduced: "Prolonged latency or reduced amplitude — may suggest central processing differences; correlate with history and other findings."
