Most of us take hearing for granted — until conversations feel harder to follow or the TV volume creeps higher. PTA is the simple, painless, gold-standard test that reveals exactly how well you hear, and where any difficulty lies.

What Is PTA?

PTA measures the quietest sound a person can hear at different pitches. The patient wears headphones and responds to faint beeps. Results are plotted on an audiogram — a visual map of hearing ability for each ear.

Test Specifications

Duration: 20–30 minutes
Equipment: Calibrated audiometer, headphones (air conduction), bone vibrator (bone conduction)
Preparation: Clear ear canal (checked via otoscopy)
Setting: Sound-treated room or quiet home environment

Procedure of the Test

Otoscopy check to confirm the ear canal is clear.
Headphones placed; air conduction (AC) thresholds tested first, ear by ear.
Bone vibrator placed behind the ear; bone conduction (BC) thresholds tested next.
Masking noise played in the non-test ear when needed, to prevent crossover.
Patient responds (button/hand) at the softest audible level for each tone.
Results plotted on an audiogram and explained to the patient.

Why Is the Test Done?

Detects the presence and degree of hearing loss
Identifies the type of loss — conductive, sensorineural, or mixed — via AC-BC comparison
Establishes a baseline before hearing aid fitting
Investigates symptoms like difficulty following speech or high TV volume
Monitors hearing over time, with noise exposure or aging
Supports occupational or medico-legal screening needs

What Will the Result Show?

Whether hearing is normal or shows loss, and to what degree (mild/moderate/moderately severe/severe/profound)
Type of loss, based on the gap between AC and BC thresholds
Which frequencies are most affected
Whether the loss is one-sided or affects both ears equally

Interpretation

Normal: "Hearing thresholds within normal limits (0–25 dB) bilaterally. No hearing loss detected."
Conductive Loss: "AC thresholds elevated, BC thresholds normal, with an air-bone gap present — consistent with conductive hearing loss. ENT evaluation recommended."
Sensorineural Loss: "AC and BC thresholds equally elevated, no significant air-bone gap — consistent with sensorineural hearing loss. Hearing aid evaluation recommended."
Mixed Loss: "Both AC and BC thresholds elevated, with an additional air-bone gap present — consistent with mixed hearing loss. Further ENT and audiological management advised."
