Hearing in the Real World: Understanding the Speech-in-Noise Test -By Acoustik Hearing

Quiet-room hearing tests don't always predict real-life difficulty. Many people hear fine in a silent clinic but struggle badly at a family dinner or a crowded restaurant. The Speech-in-Noise (SIN) Test measures exactly this — your ability to understand speech when background noise is present.

What Is the Speech-in-Noise Test?

The SIN test presents sentences or words against background noise (multi-talker babble), at different Signal-to-Noise Ratios (SNR) — the relative loudness of speech versus noise. The patient repeats what they hear, and the percentage correctly identified is scored at each level.

Test Specifications

Duration: 15–20 min
Equipment: Calibrated audiometer/speech-in-noise system with sentence lists and background noise
Preparation: Clear ear canal (checked via otoscopy)
Setting: Sound-treated room at the clinic
Age Group: Cooperative older children through seniors

Procedure of the Test

Otoscopy check to confirm the ear canal is clear.
Patient seated with speech and noise presented via headphones or speakers.
+10 dB SNR: Speech 10 dB louder than noise — an easier condition, like a quiet room with mild background sound.
0 dB SNR: Speech and noise equally loud — a moderately challenging everyday environment.
-10 dB SNR: Noise 10 dB louder than speech — a demanding condition, like a loud restaurant.
Percentage correctly repeated is scored at each SNR level and reviewed with the patient.

Measures real-world listening ability, beyond quiet-room thresholds
Explains difficulty in social settings despite "mild" hearing loss on paper
Guides hearing aid selection — noise-reduction and directional microphones
Verifies hearing aid benefit in noisy environments after fitting
Identifies candidates for assistive listening devices

What Will the Result Show?

The percentage of words/sentences correctly understood at +10 dB, 0 dB, and -10 dB SNR, compared to expected performance for a person's age and hearing status.

Interpretation

Good (80–100% at 0 dB): "Speech understanding in noise within expected range. Minimal difficulty anticipated in typical environments."
Reduced (50–79% at 0 dB): "Moderate difficulty understanding speech in noise. Hearing aid with noise-reduction and directional microphones recommended."
Poor (below 50% at 0 dB, or struggling at +10 dB): "Significant difficulty in noise, disproportionate to quiet-room thresholds. Advanced hearing aid technology and/or assistive devices strongly recommended."
