How Well Do You Really Hear in a Crowd? Understanding the QuickSIN Test - By Acoustik Hearing

Many people with hearing loss follow conversations fine in a quiet room, yet struggle badly at a noisy dinner table or party. A standard hearing test alone can't predict this — which is where the QuickSIN (Quick Speech-in-Noise) Test comes in, giving a fast, reliable estimate of exactly how noise affects your understanding.

What Is the QuickSIN Test?

QuickSIN measures SNR loss — how much louder speech needs to be than background noise for you to understand it, compared to someone with normal hearing. The patient repeats sentences played in multi-talker babble, growing progressively harder as noise rises relative to speech, from an easy 25 dB SNR down to an extremely difficult 0 dB SNR.

Test Specifications

  • Duration: 10–15 min
  • Equipment: Audiometer with QuickSIN capability, headphones/insert phones or free field speaker
  • Preparation: Clear ear canal; pure tone audiometry completed first
  • Setting: Sound-treated room at the clinic
  • Age Group: Adults and adolescents with solid English understanding (language-dependent)

Procedure of the Test

  1. Presentation level set — 70 dB HL if the four-frequency average is under 45 dB HL, or a comfortable "loud but OK" level otherwise.
  2. Patient instructed to repeat each sentence, guessing where needed as noise increases.
  3. Six sentences (five key words each) played at decreasing SNR: 25, 20, 15, 10, 5, and 0 dB.
  4. Correctly repeated key words scored per sentence.
  5. SNR loss calculated: SNR loss = 25.5 − total correct.
  6. Two or more lists averaged for accuracy; an aided version compares hearing aid benefit.

Why Is the Test Done?

  • Measures real-world speech understanding in noise, beyond quiet-room thresholds
  • Sets realistic expectations for noise-related challenges, before and after fitting
  • Guides selection of directional microphones or remote mic/FM systems
  • Compares unaided versus aided performance to verify hearing aid benefit

What Will the Result Show?

An SNR loss value in dB — how much extra signal-to-noise advantage the patient needs versus a normal-hearing listener to understand half of what's said.

Interpretation

  • 0–3 dB (Normal): "SNR loss within normal range. Minimal difficulty expected in typical noisy environments."
  • 3–7 dB (Mild): "Mild SNR loss — some difficulty in noise expected; hearing aid with directional microphones may help."
  • 7–15 dB (Moderate): "Moderate SNR loss — noticeable difficulty; directional microphones alone may not suffice, additional technology like remote microphones may help."
  • >15 dB (Severe): "Severe SNR loss — significant difficulty expected; advanced assistive technology strongly recommended alongside hearing aids."
