Listening to the Brain's Response: Understanding the Auditory Brainstem Response Test

Some hearing evaluations don't rely on a patient pressing a button — they measure how the brain itself responds to sound. The Auditory Brainstem Response (ABR) test does exactly this, offering an objective window into the hearing pathway from the inner ear to the brainstem.


What Is the ABR Test?

ABR records tiny electrical signals generated by the auditory nerve and brainstem in response to clicking or tone-burst sounds played through earphones. Small electrodes on the scalp and earlobes pick up these signals, displayed as a series of waves, revealing how well sound is transmitted along the hearing pathway.


Test Specifications

  • Duration: 45–90 min
  • Equipment: ABR system with surface electrodes, earphones/insert transducers
  • Preparation: Clean scalp; natural sleep preferred for infants, quiet rest for adults
  • Setting: Quiet, electrically shielded room at the clinic

Procedure of the Test

  1. Scalp and earlobes cleaned; small electrodes attached with adhesive gel.
  2. Patient rests quietly or sleeps (natural/sedated for infants), as stillness improves signal clarity.
  3. Clicking or tone-burst sounds presented through earphones, one ear at a time; electrodes detect the brain's electrical response.
  4. Responses averaged over many repetitions to produce a clear waveform, then analyzed against normal values.

  • Screening for patients unable to do standard hearing tests
  • Evaluates the nerve/brainstem pathway for retrocochlear conditions
  • Investigates asymmetric hearing loss, one-sided tinnitus, or suspected nerve tumors
  • Estimates thresholds objectively when behavioral testing is unreliable

What Will the Result Show?

A waveform showing distinct peaks (labelled Wave I through V), with their timing (latency) and size (amplitude) compared against expected normal values for the patient's age.


Interpretation

  • Normal: "Wave latencies within normal limits bilaterally — consistent with normal nerve/brainstem function and normal estimated thresholds."
  • Elevated Thresholds: "Response present only at higher intensity — suggestive of hearing loss; degree to be correlated with behavioral audiometry."
  • Abnormal/Absent Waves: "Delayed or absent components beyond Wave I — suggestive of retrocochlear involvement. ENT referral and imaging recommended."
  • Asymmetric Findings: "Significant difference between ears — warrants evaluation to rule out one-sided nerve pathology."
