Testing Hearing Before a Baby Can Speak: Understanding Visual Reinforcement Audiometry (VRA)

How do you test hearing in a child too young to raise a hand or press a button? Visual Reinforcement Audiometry (VRA) answers exactly this — a specialized, play-based hearing test for full-term infants who are at least 5 to 6 months old (Moore et al 1977) and is typically recommended up to roughly 24 months of age (ASHA 2004; AAA 2012).

What Is VRA?

VRA trains a child to turn their head toward a sound by rewarding that turn with a visual reward — a toy that lights up or an animated image on screen. Over repeated trials at decreasing volumes, the audiologist finds the child's minimum response level at different pitches.

Test Specifications

Duration: 30–45 min, sometimes across multiple short sessions
Equipment: Sound field speakers with visual reinforcer system
Preparation: Child should be well rested, fed, and not due a nap
Setting: Dedicated, sound-treated room at the clinic
Age Group: 5 to 6 months old (Moore et al 1977) and is typically recommended up to roughly 24 months of age (ASHA 2004; AAA 2012).

Procedure of the Test

Conditioning: Sound played alongside the visual reward, teaching the child to link the two.
Two "check" trials (sound alone) confirm true conditioning once the child reliably turns.
Testing phase: Sound level lowered in steps (20 dB down, 10 dB up), tracking head-turn responses.
A second tester engages the child with quiet toys, maintaining attention without cueing.
Testing repeats across key frequencies, for air conduction, bone conduction, and each ear where tolerated.
A minimum response level is set once a reliable pattern (2 of 3 responses) is seen.

Objective hearing measure for infants too young for standard audiometry
Detects hearing loss early, in the critical window for speech/language development

What Will the Result Show?

The minimum response level (in dB) at each frequency tested, for air conduction, where tolerated — for both ear simulatneously, plotted similarly to an audiogram.

Interpretation

Normal: "Responses obtained at levels consistent with normal hearing for age; no concerns identified. Routine monitoring recommended."
Elevated Air Conduction: "Responses only at raised levels — hearing loss suspected; bone conduction testing recommended."
