What Is the Stenger Test?
The Stenger
Test relies on the Stenger phenomenon: when identical tones play to both
ears at once but at different volumes, a person can only consciously perceive
the louder one. If someone is exaggerating or has a non-organic (functional)
hearing loss, they may fail to respond when the louder tone is aimed at their
"poorer" ear — because that's actually the ear they hear better, and
the loudest tone reaching them goes unacknowledged.

- Duration: 10–15 min
- Equipment: Two-channel calibrated
audiometer, supra-aural or insert earphones (not bone conduction)
- Preparation: Clear ear canal; conventional
pure tone audiogram completed first
- Setting: Sound-treated room at the clinic
- Age Group: Adults primarily; used cautiously
in children
Procedure
of the Test
- A conventional pure tone audiogram is obtained,
noting inconsistent responses or unexpected asymmetry.
- The frequency with the largest asymmetry is
selected; baselines reconfirmed in both ears.
- In the poorer ear, a tone is set 10–20 dB below
threshold; in the better ear, 10–20 dB above threshold.
- Both tones are presented simultaneously; response
(or lack of one) is recorded.
- Trial repeats for consistency; poorer ear's
intensity rises in 5 dB steps while better ear stays fixed.
- Process repeats at other clinically relevant
frequencies as needed.
Why Is the
Test Done?
- Identifies a non-organic component in unilateral or
asymmetric hearing loss
- Recommended for a large gap between ears or
inconsistent audiometry responses
- Useful in medico-legal situations where motive to
alter responses may exist
- Guides need for objective testing (ABR, ASSR, OAE,

What Will
the Result Show?
Whether the
patient responds to simultaneous tones as expected, or fails to respond despite
the louder tone reaching the "poorer" ear.
Interpretation
- Negative Stenger (button pressed):
"Patient responded appropriately — not indicative of non-organic
hearing loss. Consider further investigation or cross-check with
independent measures."
- Positive Stenger (no response): "No
response despite the louder tone in the poorer ear — indicative of a
non-organic component. Objective assessment (ABR, ASSR, OAE, or acoustic
reflexes) recommended."
