Testing the Ear's Balance Connection: Understanding the c-VEMP Test - By Acoustik Hearing

Not every hearing and balance test focuses on sound alone — some assess how the inner ear communicates with the muscles that stabilize your head and neck. The Cervical Vestibular Evoked Myogenic Potential (c-VEMP) Test does exactly this, revealing a part of the inner ear rarely evaluated by standard hearing tests.


What Is the c-VEMP Test?

c-VEMP measures the response of the saccule (an inner ear balance organ) and its nerve pathway to loud sound. Neck electrodes detect a reflex muscle response in the sternocleidomastoid muscle, triggered when the saccule is stimulated — revealing whether this part of the vestibular system functions normally.


Test Specifications

  • Duration: 30–45 min
  • Equipment: Evoked potential system with surface electrodes, insert earphones, EMG monitoring
  • Preparation: Clean neck skin for electrodes; patient must turn/hold head to activate neck muscles
  • Setting: Quiet, controlled clinic room
  • Age Group: Adolescents through seniors
  • Pain Level: None — mild neck muscle fatigue possible from sustained contraction

Procedure of the Test

  1. Neck skin cleaned; electrodes placed over the sternocleidomastoid muscle, with reference/ground electrodes positioned appropriately.
  2. Patient turns their head to the opposite side or lifts it slightly, activating the muscle being tested.
  3. Loud clicking or tone-burst sounds presented through insert earphones, one ear at a time.
  4. EMG activity monitored to ensure consistent muscle contraction.
  5. Responses averaged over multiple presentations into a clear waveform.
  6. Repeated for the other ear; results compared between sides.

Why Is the Test Done?

  • Assesses saccular and vestibular nerve function, not covered by standard hearing tests
  • Investigates unexplained dizziness, vertigo, or balance disorders
  • Supports diagnosis of conditions like Meniere's disease or semicircular canal dehiscence
  • Evaluates vestibular nerve involvement in suspected acoustic neuroma

What Will the Result Show?

A waveform showing presence, latency, and amplitude of the muscle response, compared between the two ears and against expected normal values.


Interpretation

  • Normal: "Symmetric responses present bilaterally, latency and amplitude within normal limits — consistent with normal saccular/vestibular nerve function."
  • Absent Response: "No measurable response on one side — suggestive of saccular or nerve dysfunction; further evaluation recommended."
  • Enhanced Response: "Response larger than expected or present at unusually low levels — can indicate semicircular canal dehiscence; imaging may be advised."
  • Asymmetric Findings: "Significant difference between ears — warrants correlation with other vestibular tests and symptoms."
