Once a toddler outgrows Visual Reinforcement Audiometry, a new challenge arises — how do you test a child too old for toy rewards but too young to press a button? Conditioned Play Audiometry solves this by turning the hearing test into a game.


What Is Conditioned Play Audiometry?

The child performs a simple game action — placing a ring on a stick, dropping a toy in a bucket — each time they hear a sound. The action replaces the button press used in standard audiometry. It's typically used from around 2 years, once visual reinforcement stops holding attention, and can extend to older children or those with additional needs.


Test Specifications

  • Duration: 30–45 min, sometimes across multiple sessions
  • Equipment: Audiometer with headphones/insert earphones, bone conductor, sound field speakers, play toys
  • Preparation: Child well rested and cooperative; readiness checks (dexterity, waiting skills) done first
  • Setting: Sound-treated clinic room
  • Age Group: ~2 years and older

Procedure of the Test

  1. Teaching/conditioning: The task is demonstrated — "When you hear the sound, put the man in the boat" — practiced at a clearly audible level.
  2. Independent responses: Child performs the action alone, confirming understanding.
  3. Testing phase: Volume lowered in steps (20 dB down, 10 dB up), tracking correct responses.
  4. Testing alternates between ears after one or two frequencies, then completes the range.
  5. "No-sound" trials check for false positives periodically.
  6. Threshold confirmed once 2 of 3 ascending responses occur at the same level.

Why Is the Test Done?

  • Provides ear-specific thresholds for children too old for VRA but not ready for standard audiometry
  • Assesses individuals with additional needs unable to perform conventional testing
  • Confirms hearing aid benefit through "aided" play audiometry
  • Establishes hearing loss type via air/bone conduction comparison

What Will the Result Show?

Hearing thresholds (dB) for air and bone conduction at key frequencies, for each ear individually where tolerated — plotted like a standard audiogram.


Interpretation

  • Normal: "Thresholds within normal limits bilaterally for age. Routine monitoring recommended."
  • Air-Bone Gap: "Air conduction raised with normal bone conduction — consistent with conductive hearing loss; ENT referral recommended."
  • Equally Raised: "Both air and bone equally raised — consistent with sensorineural hearing loss; hearing aid evaluation advised."
  • Incomplete/Unreliable: "Reliable thresholds not obtained this visit — further assessment recommended as readiness skills develop."
