What Is
the Test?
This test
measures whether the acoustic (stapedial) reflex is sustained normally over
time, or weakens abnormally fast. A sustained tone is played for about 10
seconds at a level known to trigger the reflex, and the equipment monitors
whether the response holds steady or decays significantly.

- Duration: 5–10 minutes
- Equipment: Immittance/tympanometry unit with
reflex decay measurement
- Preparation: Clear ear canal (checked via
otoscopy)
- Setting: Sound-treated room at the clinic
- Age Group: Mainly adults and older children
Procedure
of the Test
- Otoscopy check to confirm the ear canal is clear.
- Probe placed in the ear canal to seal it and
monitor pressure/reflex.
- A tone at a level known to trigger the reflex is
played continuously for 10 seconds.
- Reflex response is monitored throughout the
sustained tone.
- Test repeated at another frequency if needed, and
for the other ear.
- Decay pattern recorded and reviewed with the
patient.

conditions
- Used alongside Acoustic Reflex Threshold,
tympanometry, and pure tone audiometry
- Investigates unexplained asymmetric hearing loss
- Evaluates unilateral tinnitus or hearing changes

- Guides referral for advanced diagnostics like ABR
or imaging
What Will
the Result Show?
Whether the
reflex response remains stable for the full 10 seconds, or weakens
significantly within that period — indicating normal or abnormal reflex decay.
Interpretation
- Normal: "Reflex response stable
throughout testing — consistent with normal auditory nerve pathway
function at this level."
- Abnormal Decay: "Reflex response
weakens significantly within the test period — suggestive of possible
auditory nerve involvement. Further evaluation, such as ABR testing or ENT
referral, recommended."
- Inconclusive: "Reflex response could
not be reliably measured — retesting or alternative evaluation
advised."
